Active Queenslanders Industry Alliance & Ampd, are collecting the personal information in this form to:

Obtain consent for the named child/student to participate in the named activity;
Help coordinate the activity; and
Create and maintain records to capture student’s progress.

Personal information listed in the form may be shared with the Queensland Government, including the Department of Trade, Employment and Training for the purposes of evaluating the program including carrying out student surveys. The information will only be accessed by authorised staff. Your personal information will be handled in accordance with the Information Privacy Act 2009.
